About the Role:
Solara Hospice LLC is looking for a compassionate and dedicated Hospice RN to join our caring team in Norwood, MA. In this meaningful role, you'll provide expert clinical care and emotional support to patients and families during life's most important journey. Responsibilities:
- Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
- Provide skilled nursing care including pain management, wound care, and symptom control
- Collaborate with interdisciplinary team including physicians, social workers, chaplains, and aides
- Educate and support patients and families on disease progression, comfort care, and end-of-life planning
- Document clinical findings, care plans, and patient updates in accordance with hospice regulations
- Respond to after-hours patient needs and on-call situations as required
- Coordinate with community resources and facilitate smooth transitions in care settings
Requirements:
- Active RN license in the state of Massachusetts
- Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of pain management, symptom management, and end-of-life care
- Excellent communication and interpersonal skills with patients, families, and care teams
- Ability to work independently in a field-based, patient home environment
- Valid driver's license and reliable transportation required
- CPR certification and compliance with all state and federal hospice regulations

Compensation: $50.00 - $55.00 per hour
